How the math works

Each step = ~800 input + ~400 output tokens (planning + reasoning context).
Each tool call = ~1,500 input + ~300 output tokens (tool result lands in context).
Each subagent = ~4,000 input + ~1,500 output tokens (full nested session).
Low/High bands are ±25% of the mid estimate.
High-risk steps match patterns like drop table, migration, rm -rf, or pile up ≥8 tool calls in one step.
